How to Submit your Immunization Requirements
Newly Admitted Students - You will gain access to the Titan Health Portal 48 hours after accepting enrollment and paying any required fees.

Immunization Requirements:
All incoming students starting Fall 2022 are required to comply with Cal State University Systemwide Executive Order 803, including the following immunizations and Tuberculosis assessment:
- Measles, Mumps, and Rubella (MMR)
- Hepatitis B (Hep B)
- Varicella (Chickenpox)
- Tetanus-Diphtheria-Pertussis (Tdap)
- Meningococcal Disease (Serogroups A, C, Y, W-135)
- Tuberculosis Screening/Risk Assessment (TB)
- COVID-19 Vaccine Series & Booster
Public Health Records:
Student Wellness accesses immunization data from the California Immunization Registry (CAIR).
- Students who received the above immunizations in the state of California can verify their vaccination records through the Titan Health Portal and will likely NOT need to upload vaccination records.
- Students who received any of the above immunizations outside of the state of California will be required to manually upload their vaccination records into the Titan Health Portal.
(Please do not submit immunizations that already show Compliant in the Titan Health Portal.)
Exemptions:
The CSU’s Immunization requirements provide provisions for exemptions. Use this link for directions to download and upload your Exemption Form.
1. Navigate to the Titan Health Portal
Open your browser to https://hcmw.fullerton.edu/
- We recommend the Google Chrome browser
6. Check Clearances
Only update the Clearances that show "Not Compliant." After updating a Clearance, verification is required, and the status will stay "Not Compliant" until someone at Student Wellness verifies your immunization. During very high volumes, it could take 2 weeks to have your Medical Clearances verified.
IMPORTANT: If you need to show proof of any immunization, please do not forget steps 9 & 10 below.
7. Fill out the TB Risk Assessment Questionaire
Note: After answering the TB Risk Assessment Questionaire, you may be required to update the Tuberculosis Testing Clearance.
8. Update all NOT Compliant Immunization Clearances
Add the Date(s) and Vaccine name for every NOT Compliant immunization.
9. Upload Immunization Records
If you have some Not Compliant immunization clearances, then you will need to upload a copy of your Immunization Records. If everything under "Items required for clearance" is compliant, then skip this step.
From your Medical Clearances screen, scroll down to the section named Additional items NOT required for clearance. Find Immunization Record, then click Update to upload your documents.
10. Immunization Document Upload
Upload a readable document(s) of all your immunizations that need to be verified by Student Wellness.
11. Compliancy
You will need your "Overall Clearance Status" to show "Satisfied." To be fully compliant, you will need a green Compliant checkmark for each Clearance.
IMPORTANT: Your Clearance will show NOT Compliant until verified. During high volumes, it may take 2 weeks before immunizations are verified and marked compliant.
