How to Submit your Informed Consent and Privacy Practice Notice Form
Newly Admitted Students - You will gain access to the Titan Health Portal 48 hours after accepting enrollment and paying any required fees.

1. Navigate to the Titan Health Portal
Open your browser to https://hcmw.fullerton.edu/
- We recommend the Google Chrome browser
2. Enter your campus username and password. Then click Login.
- Enter your campus username and current password.
- Click Login.
3. You'll be prompted to authenticate with Duo.
Remember you can use Other options to choose another way to authenticate with Duo.
4. Enter your Date of Birth
5. Click Medical Clearances
6. Check Clearances
Click on the "Update" button next to Health Services Informed Consent and then Privacy Practice Notice.
7. Fill out the Health Services Informed Consent Form
Fill out the required section and then read the form.
8. Sign the Acknowledgment
Click "Submit Final" once all the required sections have been filled out.
9. Fill out the Privacy Practices Notice
Read and review this form.
10. Sign the Acknowledgement
Click "Submit Final" once all the required sections have been filled out.
11. Click Log Out to Exit the Titan Health Portal
Once you have filled out the required information for each Required Clearance, you can log out. Please do not forget to come back and check your Titan Health Portal Messages on the left side toolbar. This is how Student Wellness will contact you.
