How to Fill out your TB Screen-Risk Form
Newly Admitted Students - You will gain access to the Titan Health Portal 48 hours after accepting enrollment and paying any required fees.

1. Navigate to the Titan Health Portal
Open your browser to https://hcmw.fullerton.edu/
- We recommend the Google Chrome browser
2. Enter your campus username and password. Then click Login.
- Enter your campus username and current password.
- Click Login.
3. You'll be prompted to authenticate with Duo.
Remember you can use Other options to choose another way to authenticate with Duo.
4. Enter your Date of Birth
5. Click Medical Clearances
6. Locate the TB Screen-Risk Form
Click on the update button next to the TB Screen-Risk Form.
7. Fill out the TB Questionnaire
Fill out the following questions on the form and then click submit.
You have completed the TB Screening form.
You will proceed to step 8 if further action is necessary.
8. Submit Information if Further Action is Necessary
Please note that not every section needs to be filled out. Only submit the information that is required for you. Then click submit.
9. Click Log Out to Exit the Titan Health Portal
Once you have filled out the required information for each section, you can log out. Please do not forget to come back and check your Titan Health Portal Messages on the left side toolbar. This is how Student Wellness will contact you.
