Overview of Available Forms

This article covers a basic overview of the available forms on the IT Request Forms that require a Department IT Coordinator (DITC) to submit them.

Telephone, Cellular, and Wiring

These forms allow you to add, modify, and delete telecommunications and network-related equipment and services.

Zoom Phone

  • This request allows you to request a Zoom Phone softphone or physical desk phone or request an analog line.

Phone Modification

  • These requests allow you to make modifications to a phone including: changing the name on a phone/extension (display name change), updating an Outlook (Global Address List) entry, changing features (creating a shared line, add extension to a call pick up group, modify a shared line group or call pick up group, modify ACD call center queues, and create or modify a call tree/auto receptionist menu), and changing billing for a user, extension, or device.

Mobile Device

  • This request allows you to request a new/upgraded/replacement mobile phone, mobile access point (e.g., MiFi, hotspot), or iPad or to request changes to the plan associated with a mobile device.

Switches and Access Points

  • This request allows you to add, move, or remove a network switch or wireless access point.

Wiring

  • This request allows you to add, move, or delete a wiring outlet.

IT Clearance

  • Note: this request appears in multiple categories. This request allows you to notify IT that an employee has separated from your department and indicate what should happen to the IT assets and accounts that were assigned to that employee.

Schedule a Move

  • Note: this request appears in multiple categories. This request allows you to schedule a move of IT equipment including phones and computers
Computers and Handhelds

These forms allow you to request new computer-related equipment. Note: not all employees are eligible for Rollout equipment. Learn more about Campus Rollout Equipment and employee eligibility.

Apple TV

  • This request allows you to request a new Apple TV device for a conference room.

Laptop Workstation

  • This request allows you to order a new Rollout PC or Macintosh laptop computer for eligible employees.

iPad

  • This request allows you to order a new Apple iPad for eligible employees or purchase an iPad for a user in your department.

IT Clearance

  • Note: this request appears in multiple categories. This request allows you to notify IT that an employee has separated from your department and indicate what should happen to the IT assets and accounts that were assigned to that employee.

Schedule a Move

  • Note: this request appears in multiple categories. This request allows you to schedule a move of IT equipment including phones and computers
Account Management

These requests allow you to add/modify entries in the People Directory (aka Identity Management), add/modify/disable/re-enable/extend expiration of email/login accounts, and view/modify the campus online phone directory.

People Directory

  • These requests include adding a person to the People Directory and modifying a person’s People Directory entry.

Email/Login Accounts

  • These requests include requesting a new email account, modifying an email account, disabling an email account, re-enabling a disabled email account, and extending the expiration of an email account/People Directory position.

Phone Directory

  • This tab allows you to view, download, and modify the campus online telephone directory.
Software and Access

These forms allow you to request guest wireless accounts for an event and notify IT when an employee leaves your department.

Guest Wireless Account Request

  • This request allows you to order guest wireless accounts for an event being hosted on campus.

IT Clearance

  • Note: this request appears in multiple categories. This request allows you to notify IT that an employee has separated from your department and indicate what should happen to the IT assets and accounts that were assigned to that employee.
Other Services

This section covers all Other Services requests including opening a ticket, resetting an email or voicemail password, activating a data jack, scheduling a move, and picking up computer equipment.

Campus DL Access Request

  • This request allows you to request access to send to campus-wide distribution lists such as DL-Campus Employees.

Open New Ticket

  • This request allows you to submit a trouble ticket to the IT Service Desk.

Reset a Password

  • This request allows you to request that IT reset either a voicemail password or an email account password.

Activate Data Jack

  • This request allows you to request that an existing data jack in a location be activated.

Schedule a Move

  • Note: this request appears in multiple categories. This request allows you to schedule a move of IT equipment including phones and computers.
