Titan Health Incident Report Adobe Acrobat Sign Web Form

This guide will walk you through sending the Titan Health Incident Report for signatures via an Adobe Acrobat Sign web form.

This form is specific to the Titan Health office.

Workflow: 

  1. The Titan Health Staff or Student initiates the web form online.
  2. The form is sent to the Staff's Manager for signature.
  3. Once the Manager signs the form, it will be sent to the Titan Health Director for final approval and signature.

All participants will be notified via email once the form is completed.

Step 1:

Step 2:

  1. Click Start to begin filling out the form. You will need to complete section I, II, IV, V and VI.
  2. You can check how many required fields are remaining for you to complete.

Step 3:

Once you complete all required sections, click the Submit button on the bottom of the page.

Step 4:

  1. Enter the Manager's first name.
  2. Enter the Manager's last name.
  3. Enter the Manager's CSUF email address.
  4. Confirm the Manager's CSUF email address.
  5. Optional: Click Add Message to leave a private message for the Manager.

Ensure you are entering the correct email address for the Manager. If the form is sent to an incorrect email address, you must complete a new form.

Step 5:

  1. Enter the Titan Health Director's first name.
  2. Enter the Director's last name.
  3. Enter the Director's CSUF email address.
  4. Confirm the Director's CSUF email address.
  5. Optional: Click Add Message to leave a private message for the Director.
  6. Click Next

Ensure you are entering the correct email address for the Titan Health Director. If the form is sent to an incorrect email address, you must complete a new form.

Step 6:

  1. Enter your full name.
  2. Enter your CSUF email.
  3. Click Submit.

Step 7:

You may be prompted to verify your email address. Adobe Acrobat Sign will send you a link to the email you provided.

If you see this message, you MUST verify your signature via email. Otherwise, the web form will not be sent to the next signer, the Manager.

Step 8:

Open the email from Adobe Sign with the subject "Please confirm your submission of Titan Health Incident Report Form_V2". Click the Confirm my email address link in the email body.

Step 9:

A new page will open on Adobe Acrobat Sign with a confirmation message. The web form will now be sent to the Manager for their review and signature.

Step 1:

You will receive an email from Titan Health via Adobe Acrobat Sign with the subject: Signature requested on "Titan Health Incident Report Form_V2"

Click the Review and sign link in the email body.

Step 2:

In the signature request email, you can also view any private message from the form initiator, see where the signed form is routed next, or delegate another person to sign.

  1. Private message from the staff/student to you.
  2. The agreement indicates the next recipient(s) who will receive the form after you sign.
  3. You can choose to delegate another person to sign the agreement instead of you, if needed.

Step 3:

The agreement will open in your default web browser. 

Click the Continue button to access the agreement.

Step 4:

  1. Any private message that was included in the agreement for you will briefly appear.
  2. Click Start to begin reviewing the document.

Step 5:

Complete and sign section III. When clicking on the signature panel, you have different options to input a signature.

  1. Select how you want to create your signature: Type, draw, upload an image or sign via mobile.
  2. Input your signature.
  3. Optional: If you need to remove the signature, click Clear.
  4. Click Apply.

Step 6:

  1. Your signature is applied to the document.
  2. Click the checkbox to confirm that you agree to the document, the Consumer Disclosure and to utilize electronic signatures.
  3. Select Click to Sign.

Step 7:

A confirmation message will display letting you know that you finished signing the document, and it is now routed to the Titan Health Director for approval.

Step 1:

You will receive an email from Titan Health via Adobe Acrobat Sign with the subject: Approval requested on "Titan Health Incident Report Form_V2"

In the signature request email, you can also view any private message from the initiator, see the next step of the form workflow, or delegate another person to sign.

A confirmation message will display letting you know that you finished approving the document, and it is now routed to the last recipient for accepting.
